Patient's Rights

The patient has the right to receive notice of his/her rights upon admission and Teton Valley Hospital to protect and promote these rights.

The patient has the right to be admitted and to receive treatment regardless of his/her race, color or national origin.

The patient has the right to have a family member or other representative and one's physician/practitioner promptly notified of admission.

The patient has the right to be kept informed of his/her status: to be involved in the plan of care; to make informed decisions regarding care; and the right to have Hospital staff and practitioners comply with these directives.

The patient has the right to personal privacy, to receive physical, emotional and psychological care in a safe setting, and to be free from all forms of abuse and harassment.

The patient has the right to confidentiality of one's medical records and to access information contained in one's own record.

The patient has the right to be free from restraints of any kind.

The patient has the right to file a grievance with the Hospital.

We encourage you to discuss any concerns first with your caregiver.

You may also call the Idaho Department of Health and Welfare, Bureau of Facility Standards at 1-208-334-6626 or write them at PO Box 83720 , Boise , ID 83720-0036 to report your concern. If you are a Medicare patient, you may have concerns regarding your care reviewed by a Peer Review Organization (PRO).

As a patient, You are Responsible to...

  • Provide your caregiver with complete, accurate information about your health and your past medical history.
  • Ask your caregivers questions when you do not understand what treatment you are receiving or why.
  • Cooperate in a considerate, courteous manner with hospital personnel.
  • Be considerate of other patients.
  • Keep appointments in order to assure continuity of care.
  • Make appropriate arrangements to pay for services received and to comply with those arrangements.

 

Section 504 Notice of Program Accessibility

The regulation implementing Section 504 requires that an agency/facility "…adopt and implement procedures to ensure that interested persons, including persons with impaired vision or hearing, can obtain information as to the existence and location of services, activities, and facilities that are accessible to and usable by disabled persons." (45 C.F.R. §84.22(f))

Teton Valley Health Care and all of its programs and activities are accessible to and useable by disabled persons, including persons who are deaf, hard of hearing, or blind, or who have other sensory impairments.  Access features include:

  • Convenient off-street parking designated specifically for disabled persons.
  • Curb cuts and ramps between parking areas and buildings.
  • Level access into first floor level with chair access to clinic exam rooms.
  • Fully accessible offices, meeting rooms, bathrooms, public waiting areas, cafeteria, patient treatment areas, including examining rooms and patient rooms.
  • A full range of assistive and communication aids provided to persons who are deaf, hard of hearing, or blind, or with other sensory impairments.  There is no additional charge for such aids. Some of these aids include:
    • Qualified sign language interpreters for persons who are deaf or hard of hearing.
    • A twenty-four hour (24) telecommunication device (TTY/TDD) which can connect the caller to all extensions within the facility and/or portable (TTY/TDD) units, for use by persons who are deaf, hard of hearing, or speech impaired.
    • Readers and taped material for the blind and large print materials for the visually impaired.
    • Flash cards, alphabet boards and other communication boards.
    • Assistive devices for persons with impaired manual skills.
